В настоящее время когнитивная лингвистика расширяет свое научное поле, заимствуя и адаптируя понятийно-­терминологических аппарат исследований, совершенствуя методики анализа научного объекта, варьируя выбираемые подходы, систематизируя знания по определенным аспектам научного объекта с обособлением новых научных направлений. Также наметилась тенденция в нелингвистических или смежных с когнитивной лингвистикой научных направлениях включать язык и речь в научно-исследовательское поле. Теоретико-методологическая взаимо­связь таких направлений, как когнитивная лингвистика, психофизиология, кардиология и патопсихолингвистика, обусловила необходимость обособления клинической лингвистики как отдельного направления в рамках когнитивистики. Цель – представить обзор и систематизацию исследований отечественных и зарубежных ученых за период 1981–2022 гг. по вопросам речевых нарушений у пациентов с разного рода когнитивными расстройствами. Основу составили работы кардиологов, неврологов и патопсихолингвистов, в которых в качестве главного акцента описаны речевые отклонения. До настоящего времени вопросы речевых нарушений, обусловленные когнитивными особенностями человека, не рассматривались в рамках когнитивной лингвистики. В основном работы проводились с психофизиологических позиций, сводя исследования речевых нарушений в сугубо медицинскую плоскость, предоставляя психологам, неврологам, кардиологам, дефектологам и логопедам значимые данные, позволявшие на их основе учитывать возможные последствия для когнитивно-речевого состояния пациентов при выборе оперативного вмешательства или разрабатывать эффективные программы реабилитации таких пациентов. В нелингвистических направлениях, затрагивающих язык и речь, выявлены терминологическая общность и тенденции в исследованиях, определено сходство методик анализа. Выделено три направления исследований когнитивно-речевых нарушений: 1) связанных с периодом их внутриутробного, интранатального или раннего постнатального развития; 2) вызванных возрастными изменениями; 3) вызванных перенесенным заболеванием или полученной травмой головного мозга. Согласно полученным данным, несмотря на варьирование исследуемых языковых и речевых особенностей пациентов с когнитивными нарушениями, все аспекты языка и речи попадают в научное поле психофизиологии, кардиологии и патопсихолингвистики и выполняются в междисциплинарной парадигме. Представленный аналитический обзор позволяет говорить о некоторой терминологической несогласованности исследований, смежных с когнитивной лингвистикой, в которых рассматриваются язык и речь. Встречаются термины: речевые нарушения, речевые расстройства, речевая дисфункция, речевые дефекты, расстройства речевого развития, расстройства речевых функций, нарушения речевого развития, расстройства развития языка и речи. Кроме того, методики исследования языковых навыков и речевых умений разнятся, язык и речь исследуются при помощи разных тестов. Описаны признаки обособления клинической лингвистики в рамках когнитивных исследований для заполнения существующих теоретико-практических лакун. Центральными задачами клинической лингвистики видятся согласование понятийно-терминологического аппарата исследований, унификация методологии работ и построение единых принципов исследований языка и речи.

Cognitive linguistics, psychophysiology, cardiology, and pathological psycholinguistics have common theory and methodology. Clinical linguistics is a new area of research within cognitive science. The authors reviewed domestic and foreign studies of communicative disorders published in 1981–2022. The review focused on the cardiology, neurology, and pathological psycholinguistics of speech abnormalities, which demonstrated similar terminology and methods. The authors identified three research directions: 1) disorders associated with the period of intrauterine, intranatal, or early postpartum development; 2) age-related disorders; 3) disorders caused by a prior disease or brain injury. The language and communication profile of patients with cognitive impairments was different, but all aspects of language and communication fell into the field of psychophysiology, cardiology, and pathological psycholinguistics, thus remaining within an interdisciplinary paradigm. The review highlighted some terminological inconsistency of studies focused on the analysis of language and speech in cognitive linguistics. The authors proved that clinical linguistics is a separate branch of cognitive research that bridges a certain gap between theory and practice. They defined the main objectives of clinical linguistics as the coordination of the conceptual and terminological research apparatus, unification of methodology, and development of unified principles for language and communication research.
